Elder Care Managers, often referred to as Geriatric Care Managers, are contracted by individuals or family members to assess, coordinate, and monitor services for elderly and/or disabled individuals. Professional geriatric care managers often have advanced degrees in social work or nursing. The care manager will provide an in-home needs assessment and evaluation to determine the elderly person's strengths and needs. An elder care manager may act as a liaison to family members, doctors, banks, and community resources. Some elder care managers are members of the National Association of Professional Geriatric Care Managers or the National Association of Case Management
